By studying the rifts, Telaran scholars have so far identified six planes that interact with Telara through cracks in the Ward, each representing a primal element of the universe: Life, Death, Fire, Water, Air, and Earth. There are many theories about the true nature of these discoveries, and there is much yet to be learned. But one thing is generally agreed: The elemental planes are in danger of destroying the world of Telara and must be stopped at all costs.

Rifts are tears in the fabric of the world that allow contact with other Planes. The denizens of those Planes- whether they be Earth, Air, Fire, Water, Life, Death- invade the world, and must be stopped. These Rifts can happen anytime, anywhere. If they are not stopped, they can take over whole territories.
From the Rift Feature Guide:
A rift is like a portal through which the enemies of Telara can enter the world. Rifts dynamically show up in the world, change creatures around their vicinity, create encounters for the players, may launch attacks and may completely change a zone.
Rifts start out as a tears. A player may open a tear if they find one or given enough time, a tear will open into a rift by itself. A rift can spawn just about any where in the world, some may even spawn in town. If a rift is left alone and not dealt with by players, it may expand to cover the entire zone and/or start sending out armies.
The encounter at a rift comes in phases/waves, with each phase getting more difficult. Each phase may be different, requiring the player to deal with a new situation. There are different sizes of rifts and rifts generally spawn creatures around the level of the area in which the rift opened.
During the course of the encounter loot is automagically handed out to players that are actively taking part. If you show up late and help a little you will still get something. A group can push the rift into a unstable state (bonus type stage) to gain the best loot. It is possible to solo a rift, but you will most likely not be able to get loot from the unstable state without some help.
"A specific rift event only grows to a certain size (this may vary depending on how they play in testing) but in addition to that planar incursions can go beyond a rift event and actually travel over the lands and destroy other areas as well as other rift types."
Most randomly occurring rifts are "minor" and can be completed by a small group or a single player. Larger rifts are usually triggered by players, this reduces the chance of you walking into a large group encounter.
